What is patient2care?
People living with rare or complex diseases are often overwhelmed when it comes to finding their way around the health system. They are often lacking information on access to possible therapies, treatment centres, available support, and general information on how to better deal with the disease.
In response to that need, we are developing patient2care.
patient2care supports patients and their families with information and services all around their rare or complex diseases. It helps them to find the right answers and trustworthy supporting partners.
In its first release – which we developed in close cooperation with the German Association for Patients with Muscular Diseases (DGM) – patient2care contains information on four neuromuscular diseases: ALS, Myositis, SMA and FSHD.
It is our goal to include more disease areas onto the patient2care platform in the future, together with alliance partners.
